Driving Directions
From Downtown San Jose
Head south on S 1st Street toward W San Carlos Street. Turn right onto W San Carlos Street, then continue onto Lincoln Avenue. Our facility is located at 1351 Lincoln Ave on the right side, in the Willow Glen neighborhood. The drive is approximately 10 minutes.
From San Francisco (via US-101 S)
Take US-101 S toward San Jose. Exit onto I-280 S, then take the Bird Avenue exit. Turn right onto Bird Avenue, then left onto Lincoln Avenue. Continue approximately one mile to 1351 Lincoln Ave. Total drive time is roughly 50 to 60 minutes depending on traffic.
From San Jose International Airport (SJC)
Exit the airport via Airport Boulevard heading south. Merge onto I-880 S, then take the I-280 S exit toward Santa Cruz. Exit at Bird Avenue, turn right, then left onto Lincoln Avenue. Our center is about 15 minutes from the airport.
From South Bay (Los Gatos / Campbell)
Take CA-17 N or CA-85 N to I-280 N. Exit at Bird Avenue, turn left, then right onto Lincoln Avenue. The facility is approximately 10 to 15 minutes away.

MDCR Rehab proudly serves individuals and families throughout the San Francisco Bay Area and beyond. While our facility is located in San Jose's Willow Glen neighborhood, we welcome patients from across the region and nationwide.
